How did you hear about us?
As the parent or legal guardian of the registered child, I hereby give my permission for him or her to participate in this Key Athletics program. I understand there are risks of injury inherent in any athletic activity, including soccer. I know of no illness of physical impairment that would prevent my child from participating in this program or restrict or limit his or her participation in any of the activities involved with this program. On behalf of my child, I hereby assume all risks and hazards inherent to my child's participation in this Key Athletics program, including risks and hazards incident to transportation to and from the program. On behalf of myself and my child, I hereby release, discharge, hold harmless and indemnify Key Athletics, its officers, directors, employees, trainers, and associates, including the owners of the fields and facilities utilized for this program, from and against any liability or claim for damages related to my child's participation in this program.
I, the undersigned, agree to the following before being allowed to participate, in any way, in any Key Athletics related program or activity: I am aware that there are direct and indirect risks to me of exposure arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19), and any mutation or variation of SARS-CoV-2. I, on behalf of myself and my heirs, assigns, personal representatives, and next of kin, RELEASE, INDEMNIFY, AND HOLD HARMLESS Key Athletics, its officers, officials, agents, coaches, employees, other participants, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (RELEASEES), from any and all claims, demands, losses, and liability arising out of or related to any ILLNESS, INJURY, DISABILITY OR DEATH I may suffer while participating in or observing any Key Athletics program or activity, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
I grant permission to Key Athletics, its agents, and its employees the irrevocable and unrestricted right to produce photographs and video taken of my child, myself, and members of my family while at Key Athletics Soccer Camp for any lawful purpose including publication, promotion, illustration, advertising, trade, or historical archive in any manner or in any medium. I hereby release Key Athletics and its legal representatives from liability for any violation or claims relating to said images or video. Furthermore, I grant permission to use the statements of my child, myself, or my family members given during an interview or evaluation with or without my name for the purpose of advertising and publicity without restriction to time limit or geographic area. I waive my right, my child's rights, and my family's rights to any and all compensation stemming from the use of these materials.
